HI there, I am trying to combine and condense some of our HIPAA forms. Is there any reason the Disclose and Discuss Forms cannot be combined? Also, what patient information is required for each form?

Our HIPPA is combined. and we have the Pt’s name and dob on top only then they have to sign. Our office does everything electronically.

As with anything like this, we recommend double checking with your state guidelines to be sure you are in compliance. GOOD LUCK!
